WholePath Community Support Services
Support for Today. A Path for Tomorrow.
​WholePath Community Support Services was created to help vulnerable adults move from instability toward safety, dignity, and greater independence.
​
We believe that housing is the foundation of stability, but housing alone is not always enough. Many individuals also face barriers related to transportation, employment, healthcare, identification, benefits, personal care, re-entry, and access to community resources. WholePath was built to address those challenges through coordinated, practical support that considers the whole person and the complete path ahead.

Our primary focus is providing supportive housing for adults in Duval County who are experiencing major life transitions. We may serve individuals transitioning from homelessness, incarceration, hospitals, treatment programs, unsafe living situations, or other circumstances that have disrupted their ability to maintain stable housing.

WholePath provides more than a place to stay. We work with each resident to identify their needs, establish meaningful goals, and develop an individualized plan for long-term stability. Depending on the program and the person’s needs, support may include transportation coordination, resource navigation, life-skills development, employment readiness, benefits assistance, re-entry support, personal-care coordination, and help preparing for permanent housing.

Our approach is grounded in both compassion and accountability. We believe every person deserves to be treated with respect, included in decisions about their future, and given a fair opportunity to rebuild. At the same time, we encourage residents to participate actively in their progress, follow program expectations, and take meaningful steps toward greater independence.
We understand that every person’s circumstances are different. A veteran, an adult with a disability, a person returning from incarceration, and someone recovering from homelessness may all need housing, but they may require very different forms of support. For that reason, WholePath does not use a one-size-fits-all odel. We create individualized pathways based on each resident’s strengths, goals, risks, and support need.

We work to build strong relationships with families, caregivers, social workers, case managers, healthcare providers, re-entry professionals, veterans’ organizations, government agencies, employers, and community partners. By coordinating resources and maintaining clear communication, we help create a stronger network of support around each individual.

Our name reflects our mission. “WholePath” represents our commitment to supporting the whole person across the full journey from instability to stability.

At WholePath Community Support Services, we believe a difficult chapter should not determine the rest of a person’s story. With safe housing, consistent guidance, clear expectations, and access to the right resources, individuals can regain stability, restore dignity, and build a stronger future.
